Safeguarding Adult Reviews
A Safeguarding Adult Review (SAR) is a multi-agency process that considers whether or not serious harm experienced by an adult or group of adults at risk of abuse or neglect, could have been predicted or prevented and uses that consideration to develop learning that enables the partnership to improve services and prevent abuse and neglect in the future.
The SAR Consideration Panel of the Sheffield Adult Safeguarding Partnership Board (SASP) is responsible for recommending the commissioning of Safeguarding Adult Reviews (SARs) in line with the Care Act 2014 Guidance (Chapter 14). The SAR Subgroup is responsible for assuring the Sheffield Adult Safeguarding Partnership recommendations and actions coming out of reviews have been addressed by the partnership and individual agencies.
Safeguarding Adult Review Criteria
The SAR process is only relevant to an adult with care and support needs in the area if:
a) there is reasonable cause for concern about how the SAB, members of it or other persons with relevant functions worked together to safeguard the adult and either of the following conditions are met.
b1) The adult has died, and the Safeguarding Adult Board knows or suspects that the death resulted from abuse or neglect (whether or not it knew about or suspected the abuse or neglect before the adult died)
OR
b2) The adult is still alive, and the SAB knows or suspects that the adult has experienced serious abuse or neglect.

When making a referral please ensure that you provide as much relevant detail as possible. Referrals must be reviewed and authorised by a senior manager and sent to sasp@sheffield.gov.uk. If you have any questions please dont hesitate to contact us.
Published Learning from Safeguarding Adult Reviews
When a SAR is completed, the full report is published on our website, unless there is reason not to do so. It is published alongside a one page learning brief, to aid the dissemination of learning.
Quarterly SASP and DACT (Domestic Abuse Coordination Team) produce a SAR-DHR Newsletter, looking at themes and learning coming from reviews in the city, such a Safeguarding Adult Reviews and Domestic Homicide Reviews (DHRs). We often see cross-cutting themes across these reviews and each quarter focuses on a different theme and the relevent learning locally.
