NEW: Learning Brief Thematic Safeguarding Adult Review Tanya and Andrew (2025)

The Learning Brief for the Thematic Safeguarding Adult Review (SAR) Tanya and Andrew, has now been published and is available to read on the Learning Briefs page of the website.

Tanya was a 45-year-old white British female who died in July 2022 as a result of an accidental fall from a ninth storey window in the flat where she lived. Tanya had a background history of mental disorders, substance use disorders, and physical health problems. Tanya was also at risk of sexual and financial exploitation.

Andrew was a 67-year-old white British man who died following an assault in December 2023.  He lived alone and had a history of mental disorders, alcohol dependency and crack cocaine use. He also had a significant speech impediment and may have had a learning disability. He was both vulnerable to abuse and exploitation and had also been a perpetrator of abuse.
